Domiciliary care

Pola Well Care is an Occupational Therapist led domiciliary care service. We offer services to people with physical disabilities, dementia, mental health difficulties and people who have just returned home following a stay in hospital. We also support families who may need respite. 

The service takes a reablement approach that is person centred & goal oriented with the view to assist clients to regain or maintain independence. We use bespoke care plans that are monitored by resident occupational therapist. 

Our staff are fully trained in reablement approach to offer support to individuals with care and support needs in community settings or at home. Our services are tailored to meet individual needs on a 24-hour basis offering support with daily living activities which include; providing personal care, maintaining a habitable home environment, meal preparation, shopping, laundry tasks, companionship, and support with community access.

Our key care – reablement approach
Our care and support will try to achieve the following where possible. We will focus on: 

  • Client’s strengths to promote and maximise independence and wellbeing.
  • Encouraging promotion of self-care skills that may have been lost.
  • Supporting clients to regain or retain functional skills with the view to enable them to perform with minimal or no support.
  • Working on confidence after a spell of illness, deterioration in health, an injury, a hospital admission or an acquired disability.
  • Restoring independent functional performance.
  • Preventing re-admission to hospital or premature move to a care home.
  • Supporting clients in their own home which increases outcome in achieving meaningful goals.
  • Reconnecting people with their communities to reduce social isolation.
